Transitional rehabilitation is a program that allows patients to receive skilled care services at Doctors’ Memorial Hospital once acute hospital care is no longer required. Oftentimes, once acute hospital care is no longer required, the patient continues to need assistance not easily provided in the home. Transitional rehab is designed to assist patients in reaching their highest potential for health and independence prior to returning home.
Who Needs Transitional Rehab?
- Patients who need rehabilitation after surgery
- or illness
- Patients receiving IV antibiotic therapy
- Patients needing to regain strength and mobility
- Patients needing continued wound care
Who Pays for Transitional Rehab?
The transitional rehab program is a hospital-based program. The Doctors’ Memorial care coordinator can verify your insurance coverage. As long as a patient meets the criteria, has days left in the benefit period, and a physician approved stay, a patient may receive services.
